But is it enough just to admit these students? In The Privileged Poor, Anthony Jack reveals that the struggles of less privileged students continue long after they’ve arrived on campus. Admission, they quickly learn, is not the same as acceptance. This bracing and necessary book documents how university policies and cultures can exacerbate preexisting inequalities and reveals why these policies hit some students harder than others. Despite their lofty aspirations, top colleges hedge their bets by recruiting their new diversity largely from the same old sources, admitting scores of lower-income black, Latino, and white undergraduates from elite private high schools like Exeter and Andover. These students approach campus life very differently from students who attended local, and typically troubled, public high schools and are often left to flounder on their own. Drawing on interviews with dozens of undergraduates at one of America’s most famous colleges and on his own experiences as one of the privileged poor, Jack describes the lives poor students bring with them and shows how powerfully background affects their chances of success. Una vez más la prosa afilada y transgresora de la escritora belga mantiene al lector en vilo hasta el final, pues muy pronto el soldado norteamericano se revelacomo un maestro en construir ficciones que supera en su arte al de la misma Nothomb, destinataria de sus misivas.

Los enfermos en situación avanzada y terminal presentan una alta necesidad y demanda asistencial con implicación de todos los niveles asistenciales del sistema sanitario. El 25% de todas las hospitalizaciones corresponde a enfermos en su último año de vida, presentando además costes muy elevados. Por diversas razones, entre las que destaca una orientación predominantemente curativa de la medicina actual y una limitada formación en cuidados paliativos, la atención estándar recibida por los enfermos en fase terminal no alcanza el grado de desarrollo necesario. Se ha evidenciado la coexistencia de situaciones de encarnizamiento y de abandono terapéutico. A pesar de los importantes avances médicos de los últimos años, la atención adecuada al creciente número de personas ancianas, pacientes con enfermedades crónico-degenerativas y cáncer sigue representando un importante reto de salud pública. En España fallecen anualmente 380.000 personas, pudiendo estimarse que un 50-60% de las mismas lo hace tras recorrer una etapa avanzada y terminal. Las grandes cifras no deberían difuminar la experiencia única y trascendente de cada ser humano en el periodo final de su vida. Existe una demanda generalizada de una atención centrada en el ser humano, de calidad y a costes razonables, que permita una vida y una muerte dignas. Las prioridades de los pacientes son: tener los síntomas aliviados, evitar una prolongación inapropiada de su situación, tener una sensación de control en la toma de decisiones, no ser una carga y estrechar los lazos con sus seres queridos. Los cuidados paliativos intentan dar una respuesta profesional, científica y humana a las necesidades de los enfermos en fase avanzada y terminal y de sus familiares.

La Medicina del Dolor es un área reciente que trata de mejorar la calidad de la atención de estos pacientes. El dolor es un proceso neurosensorial cuyo tratamiento debe dirigirse tanto a la causa como a los mecanismo fisiopatológicos que intervienen. Por ello, la estructura de esta obra abarca todos los aspectos relativos a la disciplina: fundamentos de anatomofisología, evaluación del dolor, tratamientos no farmacológicos, farmacológicos e intervencionistas, diferentes cuadros clínicos de dolor y dolor en situaciones especiales. Para mejorar su comprensión, siempre que ha sido posible, se han incluido algoritmos diagnósticos y de tratamiento basados en la evidencia.

Los escritores bien reelaboran motivos tradicionales o bien estos eran fruto de la originalidad literaria, aunque estuviera apoyada en elementos folclóricos. Esta edición recoge las versiones literarias elaboradas por autores de reconocido prestigio en la historia de la literatura polaca.
